Knowing how a form behaves in the trainee app helps you build forms that get finished. This page is a short tour.

Finding the form

A form that is waiting is shown at the top of the app’s home screen with the title You have a form to fill in, or the count when there is more than one. The trainee taps Fill in the form, or Later to dismiss the prompt for now. If form notifications are on, the trainee also gets a push notification that opens the form directly. A trainee whose plan is frozen does not see forms in the app until the plan is running again.

Filling it in

The form opens page by page. Each page ends with a button that uses your Continue button label, or Next and Submit form when you left it empty. Fields match their type:
  • Number, Height and Body measurement open a decimal keypad.
  • Weight uses a ruler the trainee slides, in steps of 0.1 kg.
  • Range is a slider and Rating is a row of stars.
  • Phone has a country picker.
  • Photo and video fields upload as soon as the trainee picks a file. Photos show a preview straight away, and a failed upload can be retried.
Fields and pages with a condition appear and disappear as the trainee answers.

Autosave

Answers are saved on the phone as the trainee types. If they close the app or get interrupted, the form reopens where they left off, on the same page.
  • The draft lives on that phone only. It does not follow the trainee to another device and it does not survive reinstalling the app.
  • A draft is kept for 30 days.
  • The draft is cleared once the form is submitted.
You cannot see a half-filled form on the web. A response appears only after the trainee submits.

Leaving the form

A form can be left only with the close button at the top. Swiping back or pressing the phone’s back button does not close it, so a trainee does not lose their place by accident.

Required and invalid answers

The trainee cannot move to the next page, or submit, while the current page has a problem.
  • A message above the button says how many fields are unanswered, for example One field is unanswered, please complete it to continue.
  • Go to field scrolls to the first one.
  • Each field that needs attention is outlined in red until it is fixed.
This covers more than empty required fields. A badly typed email, an invalid ID number or a number outside your minimum and maximum blocks the page the same way. The server checks everything again when the form is submitted, using the same rules.

After submitting

The trainee sees your thank-you page, or the app’s standard message if you did not write one. On your side:
  • The form moves to Form history on the trainee card.
  • Fields with Save to client card update the trainee. See Responses.
  • An Intake form or Check-in shows up in check-in tracking.
A form can be submitted once. To collect the same answers again, send the form again.

Tips for forms that get finished

  • Keep pages short. Three or four questions per page is easier on a phone than one long page.
  • Mark a question required only when you truly need it.
  • Use Help text / explanation to say what you expect, such as “in the morning, before eating” on a weight field.
  • Test the form in Form simulator before you turn it on.
