Click New form at the top of the Forms screen. The button carries an AI tag because the same window offers both ways to start: build by hand, or hand the assistant an existing form to rebuild.

Pick the form type

The window asks Which form shall we build? Use the type slider at the top to choose Intake form, Check-in, One-Time or Document to sign. The type you pick here is the type the new form gets. You can change it later in Form settings, except for a Document to sign.

Build by hand

Click Build manually. Perform creates an empty form of the selected type and opens it in the builder.
  • The form is named after its type. Rename it in the builder.
  • A new Check-in form starts with a Weekly frequency. Change it in Form settings.
  • The form starts as a Draft.

Build with the assistant

The assistant reads an existing form and rebuilds it with its fields and pages in Perform’s format. It works for Intake form, Check-in and One-Time. You can give it any of these:
  • A link to a form in another system. The hint names Google Forms, Typeform and Jotform. When a link is detected you see Detected a link to an existing form.
  • Pasted text. Write one field per line, put options on lines that start with a dash, and write “Page 2” to start a new page.
  • Files. Click Upload an image / document to add photos or PDFs of the form.

Choose the type

Move the slider to the form type you want.

Paste a link or the form’s content, or upload files.

Run the assistant

Click Build the form with the assistant. You see four stages: Reading the link / document, Detecting fields, options and pages, Mapping to Perform’s field types and Preparing the form for review.

Review the draft

When you see Your new form is ready! the builder opens on the new draft.

Limits

Review what the assistant built

A form made by the assistant is a starting point. Check it before you send it.
  • The draft is saved as a Draft and is inactive, so it cannot be sent by accident.
  • In the builder, a tag at the top reads Built by the assistant with the field and page counts. Hover it to see the source.
  • Fields the assistant was unsure about are highlighted. The hint says the field was recognized from the source with low confidence and is worth checking. The highlight clears once you edit that field’s label, type or options.
  • In the forms list, the form carries a Created by AI badge.
When you click Publish on an assistant draft that is still inactive, the form is published and turned on in one step. It then appears in the form pickers.

Documents to sign

A Document to sign is always built by hand. Pick it on the slider and the window explains the three steps: Upload a PDF, Place text and signature fields, Send a signing link. Click Create document to sign to open the builder. See Documents to sign for the full flow.
