A Document to sign is a PDF, such as a health declaration or a membership agreement, that a trainee fills in and signs from a link. The trainee does not need the app.

How it works

  1. You upload a PDF and place fields on it.
  2. Each time you send it, you fill in the coach fields and Perform creates a personal signing link.
  3. The trainee opens the link, fills in their fields and signs.
  4. A signed PDF with every value and an audit page is saved with the response.

Create the document

On the Forms screen click New form, move the type slider to Document to sign and click Create document to sign.

Upload the PDF

The builder opens on Upload the document to sign. Upload one PDF file. Common upload problems and what they mean:
  • The document is password-protected. Remove the password and upload again.
  • We couldn’t process this document or The document is too complex to process. Save it again as a simple PDF, for example with Print to PDF, and upload the new file.
  • The document’s pages are too large. Re-save the PDF at a standard page size.

Place fields

Two kinds of field can go on the document. Drag a field to its spot on the document, or click it to add it to the page in view. Then select it to set its options:
  • Who fills it. Pick Trainee, when signing or Coach, when sending.
    • Trainee fields are filled in when the trainee opens the signing link.
    • Coach fields are filled in by you on every send. The trainee sees the value on the document and cannot change it. Use them for things like the plan name, price or start date.
  • Field label. For a trainee field it is shown to the trainee next to the field. For a coach field it is shown to you in the send dialog.
  • Required field. A required coach field blocks sending until it is filled. A required trainee field or signature blocks signing until it is done.
  • API field key. The field’s name when the document is sent through the API or an automation. Use lowercase English letters, digits and underscores, starting with a letter, up to 40 characters. Each key must be unique.
A signature is always the trainee’s. It cannot be set to be filled by the coach.
Changing an API field key breaks automations that already use it. Leave keys alone once a document is in use.
The panel Fields on the document lists everything you placed, split into Coach fills when sending and Trainee fills when signing.

Replace or remove the PDF

  • Replace document swaps the PDF and keeps your fields in place. If the new file has fewer pages, fields on the missing pages are deleted, and you are asked to confirm first.
  • Remove document takes the PDF away but keeps your fields. They return to their spots when you upload a document with the same pages.

Publish

A document must be published before it can be sent. Publishing is blocked until:
  • a PDF is uploaded (Upload the document before publishing),
  • there is at least one field for the trainee to fill or sign (Add at least one field for the trainee to fill or sign),
  • every API field key is valid.
After publishing, check that Active is on in the Forms list.

Open the send dialog

On the trainee’s card, open the Forms tab and click One-time form.

Documents are in the form list, marked Document to sign.

Fill in the coach fields

Under Details to fill in before sending, fill in each value. These are printed on the document and the trainee cannot change them.

Create the link

Click Create signing link.

Send it

In The signing link is ready, use Copy or WhatsApp to send the link to the trainee.
If the document is not ready, the dialog tells you why and offers Open in the form builder:
  • This document is still a draft. Publish it in the builder before sending.
  • No PDF has been uploaded to this document yet.
  • This document has no field for the trainee to fill in or sign.

To several trainees

On the trainee board, tick the trainees, click Send form and pick the document. The values you type in the coach fields are printed on the document for every selected trainee. Click Create signing links. Each trainee gets a personal link. Send them one by one, or click Copy all links to copy every link with the trainee’s name next to it.
The document never appears in the trainee app and no push notification is sent. The trainee only gets it when you send the link, so do not skip that step.
The WhatsApp button opens a chat with the trainee’s number and the link already typed. If the trainee has no phone number, WhatsApp asks you to choose the chat.

What the trainee sees

The link opens the document in the browser with a short greeting and the instruction to fill in the highlighted fields and sign where marked.
  • Coach values are shown as Prefilled.
  • A counter shows how many required fields are left, then Ready to sign.
  • Tapping a signature box opens Your signature, where the trainee signs with a finger. They can Clear, Undo last stroke and then Use this signature.
  • Sign and send submits the document. By tapping it the trainee confirms they read the document and that the signature is theirs.
After signing, the trainee sees Document signed and can use Download the signed document. Other screens the trainee may see:

Track and view signed documents

On the trainee’s Forms tab an open request shows Awaiting signature. You can copy the link again or unsend the request. Unsending removes the link for good, and the trainee then sees Link not found. Once signed, the response appears in Form history. Click View to open it. The Signed document section lets you Open, Download or view the PDF in Full screen, and shows when it was signed and from which IP address. A signed document is a record, not a questionnaire. It does not go to check-in tracking and no AI summary is written for it.

Editing a document that is in use

Each signing link keeps the fields and coach values it was created with. Editing the document later changes new links only. Links that are already out keep working as they were sent.
