The builder opens when you create a form or click a form in the Forms list. It fills the screen and has a top bar and three panels.

Layout

The field list and the edit panel need a wide screen. On a narrow window only the canvas is shown, so build forms on a desktop or laptop.

Add and arrange fields

  • Add a field. Click it in the field list, or drag it onto the canvas. It is added to the page you are viewing.
  • Select a field. Click it on the canvas. Its settings open in the edit panel.
  • Reorder. Drag a field to a new position, or use Move up and Move down.
  • Copy. Click Duplicate. The copy is placed right under the original with the same settings.
  • Remove. Click Delete on the field, or Delete field at the bottom of the edit panel.
The field list is grouped into Basic fields, Identity & metrics, Selection & rating, Media, Trainee photos and Display. See Field types for what each one collects.

Settings every question has

Select a question to see these in the edit panel.
  • Field label. The question the trainee reads.
  • Help text / explanation. Optional short text shown below the field.
  • Required field. When on, the trainee cannot continue without filling it in. Every question has this switch except Food preferences, because “nothing excluded” is a valid answer there.
  • Save to client card. Shown only for fields whose answer can update the trainee’s card or tracking. It is on by default. See Responses.
  • Condition. Show the field only when an earlier answer matches a rule. See Conditions.
Display blocks such as Text and Image / video have their own content settings and no label, help text or required switch.

Pages

A form can have several pages. The trainee moves through them with a button at the bottom.
  • Click New page to add a page. It opens right away.
  • Click a page tab to edit that page.
  • Open Page actions on a tab to set a Page condition or to Delete page. Deleting a page removes every field on it. A form always keeps at least one page.

Page settings

Click an empty area of the canvas so no field is selected. The edit panel shows Page settings with one option:
  • Continue button label. The text on the button at the bottom of this page. Leave it empty to use Next, or Submit form on the last page. The Previous button appears by itself from the second page.

Thank-you page

The Thank-you page is what the trainee sees after submitting. Open it from the canvas to edit the message. You can bold text, change colours and add new lines. If you never edit it, the trainee sees the app’s standard thank-you message.

Form settings

Click the settings button next to the form name to open Form settings. The frequency and send day on a check-in form are defaults. They fill in when the form is linked to a trainee, and you can change them per trainee. See Check-in schedule. Click Done to close the window. Settings are stored when you save or publish the form.

Test with the simulator

Click Form simulator in the top bar. The form opens the way a trainee sees it, with the badge Simulator mode, filling it in like a trainee. Fill it in page by page to check required fields, conditions and page order. At the end you see Form sent! and This is a simulation, nothing was saved. Click Fill again to start over.

Save and publish

  • Save draft stores your changes. The form’s status and version stay as they were.
  • Publish stores your changes, marks the form Published and raises the version by one.
A form needs a name before it can be saved. Without one you see Please enter a form name. If you try to leave with unsaved changes, Perform asks whether to save them first.
Publishing does not turn a form on. Check the Active switch in the forms list before you expect to send it. The one exception is a draft built by the assistant, which is turned on when you publish it.

Editing a form that is already in use

Each submitted response keeps an exact copy of the questions it answered, so old responses always display the way they were filled, even after you change the form. Forms that were sent but not yet filled are different. The app loads the form as it is saved right now, not as it was when you sent it.
Any change you save reaches trainees who still have the form waiting, and that includes changes saved with Save draft. If a form is in use, finish your edits in one sitting, or duplicate the form, edit the copy and switch over when it is ready.
A document to sign works the other way. Each signing link keeps the fields and coach values it was created with, so later edits only affect new links.
